Overview
Dr. Mundo's whole design is a bet: that he can stack more health than the enemy team can remove, then heal back the difference. He pays for spells with health instead of mana, throws cleavers that deal percent-health damage and heal him, and his passive periodically shrugs off one crowd control effect. His lane starts slow. Early Mundo is genuinely beatable, since his cleavers cost health and his items have not arrived, but every level closes the window. By mid game he trades without caring, and by late game his ultimate heals back damage that took a whole teamfight to deal. His counters are famous: Grievous Wounds, percent-health damage, and kiting. Everything else only delays him.

- Ornn: His combo stings but cannot keep pace with your sustain over a full lane, so his whole trading pattern loses to your regeneration.
- Shen: Shen's block stops autos you barely throw, and his taunt drags a champion who does not mind being dragged, so the lane stalls out and your scaling wins it.
- Renekton: All his damage is front-loaded fury, and you are built to eat a burst and keep walking. He spikes first and falls off, while your sustain keeps growing.
Early Game
Watch the cost: every cleaver spends health, and your items are not there yet to cover it. Poke with Q when the heal on hit makes the trade worth it, soak what you must behind your regeneration, and bait out the enemy's key crowd control against your passive before committing to anything. You have no escape, so respect ganks. The whole early game exists to set up your mid game; do not throw it away by over-fighting.
Mid Game
Your health items and ultimate arrive, and the lane quietly stops being winnable for them. Trade freely now: cleave, walk forward, swing, and let your regeneration outpace their damage. In skirmishes you are the frontline that never leaves, and your ult mid-fight undoes their best burst window. Push this hard before the enemy buys Grievous Wounds; there is a window where they simply have no answer to you.
Late Game
Walk at the most important enemy and dare their team to stop you. Slow the kiters with Q, swing the big bonesaw, and save your ultimate for the moment their burst commits, so you heal through damage that should have killed you. Track enemy anti-heal items honestly: with Grievous Wounds on the field you are far easier to kill and should position more carefully. Without it, their team has no answer for you.
Teamfights
Walk at their carries and refuse to die. Front the fight, soak everything, cleave the carries trying to kite, and let your team play aggressive behind a frontline that regenerates. Pop the ultimate when their damage peaks, not before, so the heal erases their best window. You do not need kills; you need their five players spending the whole fight on someone who will not die while your four kill them.
