Overview
Milio is the support who extends your ADC's attack range and keeps them healthy. His fire kick knocks back and slows what it hits, his campfire heals everyone in it while stretching their range further, and his shield-and-cleanse ultimate strips the crowd control off your whole team the moment a fight tries to lock them down. Most enchanters keep your carry alive; Milio also changes the range your carry fights at. In the Dragon lane he wins by attrition. He out-heals poke, his soft knockup peels a melee off your marksman, and the range extension lets your ADC trade from a step further back than they should. He has no hook and no hard lockdown, so he cannot start fights, but when the enemy all-in finally comes, his cleanse turns their best engage into a wasted cooldown and your carry kites out untouched. The catch is that everything he does is reactive and his own body is easy to kill. Slow, fragile, no escape: a hook or flank that reaches him is not cleansed, it is a kill. Roams punish him because he cannot answer a pick across the map, and if he gets caught, the carry he was protecting is suddenly alone.

Early Game
Play the early lane patiently, because your sustain lets you. Put the fire spirit on your ADC for the range and the slow when the enemy steps up to last hit, so your carry chips them from outside their reach and backs off before the return trade lands. Use the campfire to win exchanges your marksman could not win on raw stats, healing the poke off both of you. Do not overspend mana early or you become a defenseless body babysitting a wave. Above all, ward the river and lane brushes and track the enemy jungler, because a Milio caught by a gank has no dash and no answer.
Mid Game
Mid game your protection starts shaping the map. Keep your ADC spaced and healthy through skirmishes, and look for the moments your ultimate flips a fight: a cleanse on your carry the instant the enemy engage commits turns their all-in into your free kill window. Around the first dragon and herald you are the reason your duo contests at full health, so set the campfire near the pit and stretch your team's range over the objective wall before the fight breaks out. You cannot start the play yourself, so attach to a teammate who can and make their engage survive the answer. Be careful roaming: every step away from your carry leaves them in a 1v1 they were never meant to fight.
Late Game
Late game Milio is why a hypercarry survives fights that should kill them. Your healing and shielding can pull a focused ADC back from near death, and your range extension lets them attack from outside the danger zone. The ultimate is your most important button: hold it for the cleanse that frees your team from a big engage, not the first shield you feel tempted to throw. Stay glued to your carry and behind your frontline, because the assassin or hook that reaches you ends the fight before your cleanse fires. Your whole job is keeping the right body alive, and an out-of-position Milio is a dead support whose carry dies next.
Teamfights
Be the answer to their engage, not the start of yours. Sit on your most important carry, extend their range so they deal damage from outside the scrum, and read the enemy commit so your cleanse-and-shield ultimate lands the instant their lockdown tries to stick. The fire spirit's slow and soft knockup can peel one diver off your backline, so spend it on whoever jumps your carry rather than on chip. The worst mistake is getting caught yourself: you have no escape, so a flank that finds you skips everything your kit can do. Stay behind the line and hold the cleanse for the moment the fight actually turns.
